1. USE
   
   
   • All lists purchased are for one-time use. They may not be reused or copied.
   • A copy of the mailing piece must be provided with the order.
     
     
2. PRICE
   
   
   • AHRA's full membership list is available for $200 per thousand.
   • AHRA's membership listing by specific state(s) can be purchased for $275 per thousand, plus a $40 set-up fee.
     
     
3. FORMAT
   
   
   • Mailing lists can be provided on a 3.5" PC disk.
     
     
4. SORT
   
   
   • Lists can be sorted by Zip code, member's last name, or by member's institution/company.
     
     
5. SELECT
   
   
   • Lists can be selected by United States, by US and Canada, by the full list, or by selecting specific states
     
     
6. DELIVERY
   
   
   • AHRA will ship orders within 10 working days of receipt of order. Rush delivery is available at an additional charge.
     
     
7. ORDERING
   
   
   • The AHRA membership Mailing List Order Form should be filled out, and submitted via the Internet, fax or by mail.
   • Orders for the membership list should include format, sort and selection requirements.
     
     
8. PURCHASER
   
   
   • Agrees to these terms and conditions which are part of the order and agrees to reimburse AHRA for all court costs and attorney's fees resulting from misuse of AHRA mailing lists.
